Note that Sunday afternoons and evenings are all that we usually have available. If you also want to be the speaker at our Sunday service on that day you should first check with our Speaker Coordinator, and then come back and fill out this form when you have your speaking date. What other dates will work for you if the one above is not available? What hours do you want? Most workshops start at 2:00 PM to give people time for lunch after the Celebration service. Be sure to include whatever time you need for setup and takedown. Deposit/Facilitation Fee amount: $ $10 per hour, $30 minimum. This is non-refundable within 30 days of the event. It is not a donation (unless you cancel within 30 days but prior to the day of the event) and will be used to compensate our facilitator for his or her time. How much will you charge for admission? Will you make exceptions or use a sliding scale? (explain if yes) What percentage of the proceeds will you donate to The Celebration? % This is in addition to the above deposit/facilitation fee. We suggest 20%, but it is up to you. If you want to offer a different arrangement, such as a flat fee, put zero here and describe your offer in the comments section below. I understand that a Celebration facilitator will be present on the premises during my workshop, and that he or she may choose to attend for free. I agree to pay any balance due The Celebration, based upon the percentage indicated above and any other agreements or arrangements that may have been made, to said facilitator at the end of the event. (The facilitator will have a remittance form and will give you a receipt.)
